Although bad breath does not require immediate medical attention, it has a major impact on social interaction. Some people may be used to their own odor and have no idea why others are intentionally trying to avoid close contact. On the other hand, the ones aware of their own bad breath may refrain from speaking in public, leading to a loss of confidence. Bad breath should not be taken lightly since it profoundly affects personal relationships, social interaction, and even the quality of life.

According to Odai, Emeka & Azodo[2], more than half actually learn about their bad breath from others including partners, friends, family members, or coworkers. Olfactory fatigue may prevent individuals from realizing the oral health problems and a few methods to determine if your breath smells are listed as the following:

1. Cup your hand over your mouth and nose and breathe into it to tell if your breath smells foul.

2. Gently run a spoon across your tongue and smell it.

3. Use floss or interdental brush in and out between your teeth then smell it to check.

4. Lick the inside of your wrist or the back of your hand and smell it after 10 seconds.

5. Ask someone who is not afraid to speak the truth to stand close and smell your breath.

Congratulations if you do not have bad breath and just keep up the good work. However, there is no need to be upset if you are a victim of bad breath since bad breath could be treated. Figuring out the reasons behind it together with taking the appropriate actions could easily get rid of the social handicap – bad breath and have better social relationships.

What are Intra-Oral Halitosis and Extra-Oral Halitosis

A number of reasons contribute to chronic bad breath and figuring out the cause is the first step in taking measures to treat bad breath. Generally speaking, bad breath is subdivided into intra-oral and extra-oral halitosis, depending on the place where it originates. In the case of intra-oral halitosis, the source lies within the mouth and extra-oral halitosis has origins outside the mouth such as underlying health conditions like gastrointestinal disorders.

1. In the morning after waking – Bacteria tend to thrive the most at night when the saliva production reduces and the salivary flow becomes low. Saliva plays a significant role in lubrication, food breakdown, and the maintenance of oral health. Therefore, reduced saliva production decreases the ability to self-manage oral hygiene, which further leads to bad breath. Morning breath could usually be treated through mouth rinsing and tooth brushing. The unpleasant smell due to dry mouth could also be eliminated by drinking more water and brushing.

2. Consumption of the foods with strong flavor – Onions, garlic, and chives have distinctive flavors due to the presence of sulfur compounds in them, which tend to mingle with gas-emitting bacteria and lead to bad breath. Drinking green tea and brushing right after eating could effectively eliminate mouth odor.

3. Caused by cavities, periodontal disease, and gingivitis – Plaque buildup on the teeth contributes to gum diseases. Food particles can get stuck between teeth, forming plaque and becoming a breeding ground for the bacteria responsible for the mouth odor. These bacteria further develop pockets between teeth, which leads to even more plaque buildup and worse bad breath problem. Since these dental problems usually lead to persistent bad breath, proper treatment is required.

4. Tongue coating – Tongue coating is comprised of food particles and bacteria. When the coating gets too thick, the bacteria will actively break down the food particles and cause odors. Therefore, removal of tongue coating results in reducing bad breath.

To put it in simple words, the cause of mouth odor is the foul smelling volatile sulfur compounds generated from the breakdown of protein by anaerobic bacteria. The volatile sulfur compounds found in mouth include hydrogen sulfide (H2S), methyl mercaptan (CH3SH), dimethyl sulphide (CH3)2S, allyl mercaptan, dimethyl disulfide, and dimethyl trisulfide and the first two are the most common ones. Therefore, consuming more foods rich in sulfur-containing compounds makes the breath stink to a greater extent. In order to prevent mouth odor, avoid keeping food particles and bacteria for a long period of time. Ways to fight bad breath include practicing good oral hygiene, keeping teeth and gums healthy, drinking lots of water, proper brushing and flossing, and cleaning tongue regularly. Alternatively, consuming the foods that could fight the growth of bacteria and help reduce sulfur compounds such as green tea may help remedy the condition.

Extra-Oral Halitosis

Extra-oral halitosis may indicate a sign of underlying health condition and the most common cause goes to the gastrointestinal disorders induced by helicobacter pylori. In order to survive the gastric juice, H. pylori form the ammonia protective layer which produces a strong-smelling odor. In addition, H. pylori affect digestive function and cause delayed stomach emptying, which result in bloating and bad breath. Therefore, the condition of extra-oral halitosis will significantly improve once the H. pylori are completely removed.
